Senior Outreach is a program of Mercer Island Youth & Family Services. This program provides the following services to and for senior adults ages 55 and older and their family members.
In-Home Assessment & Case Management
Our specialist provides in-home assessments to determine the individual needs of each senior adult. Linkage and coordination of care with primary physicians and other health or social services professionals are also provided. Follow-up visits and ongoing services can be provided as needed.
Grief and Loss Support
One on one support and counseling for senior adults 55 and older.
Our specialist provides individual and couples counseling to senior adults to help manage retirement issues, depression, loss, and other life changes.
Information & Resources
Our specialist has knowledge about a wide range of services for senior adults. Mercer Island seniors and their families can access information about resources such as housing, caregivers, transportation, chore help, respite care, senior day programs, and health care. We now have an Alzheimer's and caregivers resource section in our library. Come to the office and help yourself.
Consultation Services for Adult Children
Our specialist provides consultation services to caregivers and family members to assist them in developing plans of care that meet a senior adult's individual needs.
Visit our Caregiver Corner for a wealth of resources. The Corner is located on the second floor of the Luther Burbank Park Administration Building. You can browse our resources during normal business hours. Provided by a grant from the Senior Foundation of Mercer Island, the "Corner" is a place where caregivers of senior adults can learn about community services and resources. Caregivers can also check out educational books or consult with the YFS Geriatric Specialist.
Health Promotion Classes
A variety of classes are offered through the Parks and Recreation Guide to educate and promote the health and well-being of our senior adults.
